Self Assessment Form


Purpose  

The self assessment form is designed to support reviewees in preparing for their review.  However, it is no substitute for the personal interaction and discussion between reviewee and reviewer. 

The self assessment form is prepared by the reviewee in advance of the review meeting.  It provides the reviewee with an opportunity to reflect on the past year and look ahead at what contribution they might make in the year ahead. 


Guidance

The self assessment form will usually include sections that will prompt the review to think about and write down their thoughts regarding the following areas:

  • review of general progress
  • review of the job role
  • performance in relation to defined performance objectives / standards and development objectives
  • potential performance and development objectives.

In some departments / faculties the self assessment form is submitted to the reviewer in advance of the annual formal review meeting and in others it is not.  Please refer to the library of schemes for more information or your line managers.

In some departments / faculties the self assessment form is considered a formal record and is kept with the summary review form which is completed after the annual formal review meeting.  In other departments it is not.   Please refer to the library of schemes for more information or your line managers.
